Family report · 5 minutes

Tell us what is happening at MDC Brooklyn.

A short paper form, one family at a time. No names. No case numbers. You choose what to share, and you can say no to any part and still get help.

How it works

At the Saturday tent

A trained volunteer gives you the form at a separate table, away from the line, and can help you read it. They never ask for your name.

You decide, in three boxes

Use my answers in anonymous numbers. Quote one sentence without details. Contact me later (optional, kept apart from your answers). If you tick none, we do not use your report.

Your code

Your report gets a code, not a name. If you ever want to take it back, tell us the code. Without the code we cannot find it.

What we do with it

We count, month by month, and share the numbers with lawyers and oversight bodies. Reported by families, not verified. Small numbers, not representative.

What we never ask

Names. A-numbers or case numbers. Immigration status. Medical diagnoses. The unit or floor. Exact dates. Photos of you or of the form. What we never collect, nobody can lose.

Download and print

Prefer to read it first, or fill it in at home? Print it and bring it to the tent. There is no online version.
